総頸動脈圧迫椎骨動脈撮影および神経内視鏡にて診断したenlarged infundibular dilatationの1例

A Case of Enlarged Infundibular Dilatation Diagnosed by Vertebral Angiograms with Carotid Compression and Neuroendoscope

　総頸動脈圧迫椎骨動脈撮影および神経内視鏡が，内頸動脈瘤とenlarged infundibular dilatation（EID）の鑑別に有用であった1例を報告する。症例は57歳女性，MRAにて左内頸動脈瘤が疑われたため脳血管撮影を施行した。左総頸動脈撮影，椎骨動脈撮影では後交通動脈の描出が不十分だったが，総頸動脈圧迫椎骨動脈撮影にて動脈瘤遠位端から後交通動脈が分岐していることが示唆された。術中，顕微鏡下では後交通動脈の確認は困難だったが，神経内視鏡下にて後交通動脈の全長，内頸動脈接合部を確認，EIDと診断した。本症例の診断における総頸動脈圧迫椎骨動脈撮影と神経内視鏡の有用性について検討した。

A case of enlarged infundibular dilatation diagnosed by vertebral angiograms with carotid compression was reported. A 57-year-old woman suffered from head-ache. She underwent MRA which revealed left internal carotid artery aneurysm. Left common carotid angi-ograms suggested a left saccular internal carotid ar-tery aneurysm (4mm×6mm) without a posterior communicating artery. However, it was confirmed to be an enlarged infundibular dilatation on left vertebral angiograms with carotid compression. It was con-firmed at operation microscopically and neuroendo-scopically, and the dilatation was treated by coating.
