高齢者脳血管障害症例の転帰の検討

The Outcome of Cerebrovascular Disease in the Elderly

　脳血管障害患者の予後を70歳以上の高齢者群と70歳未満の非高齢者群で比較検討した。対象は，当院救命救急センターに1年間に入院した脳血管障害患者207例で，高齢者群51例，非高齢者群156例である。症例は脳出血，脳梗塞，くも膜下出血に分類し，退院時と6カ月後の状態をGlasgow Outcome Scaleで評価した。退院時，高齢者は非高齢者より重度の障害を有している症例が多かった。退院後の6カ月間に非高齢者群では状態が改善した症例が多かったが，一方，高齢者群では逆に増悪した症例が多かった。退院時および6カ月後の死亡率は，高齢者群は17.6％，52.0％であり，非高齢者群ではそれぞれ13.5％，18.8％と大きく異なっていた。脳血管障害においては，症例の年齢，退院時の状態が予後を決める重要な因子であると考えられた。

The outcome of cerebrovascular disease in the elderly, defined as persons 70 years of age and over, and in the non-elderly, those under 70 years, was compared. Fifty-one consecutive elderly and 156 non-elderly patients were studied. The patients in each group were classified according to type of cerebrovascular event : intracerebral hemorrhage, cerebral infarction, or subarachnoid hemorrhage. Patients were evaluated by using the GlasgowOutcome Scale both at the time discharge and 6 months later.

The elderly patients were more severely disabled at the time of discharge than the non-elderly patients.
