　脳梁形成不全およびベルガ腔を合併した両側内頸動脈欠損の1例を報告する。症例は47歳男性で幼少期より精神発育遅延があり施設に入所していたが，めまい発作の精査目的にて入院となった。頭部CTおよびMRIでは両側側脳室後角の選択的拡大，両側頸動脈管の欠損，脳梁の形成不全，ベルガ腔，および脳萎縮を認めた。脳血管撮影では，両側の内頸動脈は欠損しており，右椎骨動脈は低形成で，頭蓋内の血行はすべて拡張した左椎骨動脈と脳底動脈を介して供給されていた。また，眼動脈の血行は中硬膜動脈の分枝より供給されていた。両側内頸動脈欠損の報告は稀であり21例をみるのみで，多くはくも膜下出血や痙攣，脳虚血発作などで発症しているが，形態上では脳梁形成不全やベルガ腔を合併した報告はなく，精神発育遅延の原因を考えるうえでも重要な症例と思われた。

To date, only 21 cases of bilateral absence of the internal carotid artery have been reported. The authors report a case of bilateral internal carotid artery agenesis combined with corpus callosum hypogenesis and cavum vergae.

A 47-year-old man was admitted to our hospital complaining of occasional vertigo. Physical and neurological findings were normal except for severe mental retardation.
