　88歳の女性でADH分泌不適合症候群（SIADH）を合併し，剖検によりGANS（中枢神経系肉芽腫性血管炎）と診断し得た症例を経験したので報告した。88歳女性が意識障害で発症し，経過中著明な低Na血症を認めた。水分制限，Na負荷，ステロイド療法に反応せず，末期に敗血症を合併し死亡した。剖検によりGANSと診断し得たので，その臨床病理学的特徴について文献的考察を加え報告した。

We report an autopsy case of granulomatous angiitis of the central nervous system (GANS) complicated by the syndrome of inappropriate antidiuretic hormone (SIADH).

A 88-year old female was admitted because of progressive mental deterioration, fever, and vomit-ing. A computed tomogram disclosed bilateral periventricular lucency, and a low-density area in the right occipital lobe. Laboratory studies during her hospital stay, revealed hyponatremia, hypoal-buminemia, and increased antidiuretic hormone.
