　初老期発症で緩徐に進行する記銘力障害を主徴とする1臨床例を報告した。症例は69歳男性。59歳時に健忘で発症し記憶障害は緩徐に進行した。発症後10年経つが記銘力障害が突出しており，時間の失見当を除いて見当識障害や大脳巣症状はなく，ごく軽度の人格水準低下がみられた。WAIS-Rでは，言語性IQ88，動作性IQ 83，全検査IQ 85であった。頭部CTで両側側頭葉の萎縮，頭部MRIで両側側頭葉と海馬の萎縮が認められたが，画像上石灰沈着や脳管性障害は認められなかった。SPECTでは側頭葉だけでなく前頭葉の血流低下が示された。Acetazolamide負荷SPECTでbaselineでの血流低下が改善したことから血管性病変よりむしろ脳組織の変性が示唆された。

　初老期発症で緩徐に進行する記銘力障害を主徴とする症例の報告はきわめて稀である。Alzheimer病や海馬性痴呆との関連で本症例は重要である。

We report a patient with a ten-year history of slowly progressive recent memory decline without additional cognitive impairment in presenility. Aright-handed Japanese barber first experienced for-getfulness at the age of 59. Neurological examina-tion at age 64 revealed no abnormality except for severe impairment in memorizing. Brain CT-scans were normal. In spite of gradually deteriorating memory, he was capable of organizing his work until the age of 67 years.
