Extravasation陰影を呈した幼児急性外傷性硬膜外血腫の1例

ANGIOGRAPHIC EXTRAVASATION OF CONTRAST MEDIUM IN A CASE OF TRAUMATIC EPIDURAL HEMATOMA

Ｉ．はじめに

　頭部外傷急性期患老の診療において，とりわけ意識障害の存する例に対しては，なりよりもまず頭蓋内血腫の存在の有無を知ることが大切であることは言うまでもない。その際，適確な神経学的所見の把握とともに，補助診断法としての脳血管撮影は不可欠のものとなつている。この手技の普及に伴い，extravasation陰影に関する報告も次第に増えつつある。

　extravasation （以下EVと略する）とは，血管の破綻により血液が血管外に漏出する現象で，血管撮影により，たまたまレントゲン学的にEV陰影として捉えられることがある。

The formation of acute traumatic epidural hema-toma is caused by hemorrhage from the torn me-ningeal artery into the epidural space.

This fact is generally recognized, but rarelly iden-tified by extravasation of contrast medium during cerebral angiography.

The authors reported a case of traumatic epidural hematoma in which a notable extravasation of con-trast medium had been occasionally obtained during cerebral angiography.

From the study of our case, the authors have discussed about relationships between the extravasa-tion shadow, bleeding volume, and supposed factors of increasing hematoma.
