The case presented here concerned a male aged 56 who was born in Yamanashi Prefecture in Japan. For 6 years to his death, the patient had been suffered from occasional headache and frequent attacks of generalised convulsion, and subsequently followed by speech disturbance, hemiplegia on the right side, epsodes of disturbed consciousness repeated during fever, muscular twitching of right half of the body and gradual occurence of mental deterioration. The EEG illustrated a spiking focus on the right frontal area.

Pathological findings we obtained through post-mortem examination are a number of eggs of schisto-soma japonicum were found in the meninges as well as in the cerebral cortex, some of which were in the white matter, along with arteriosclerosis on the brain. The brain demonstrated various kinds of tissue reac-tions around the eggs; round cell infiltration, granu-loma formation, encephalitislike degeneration, loss of neurons and so forth. Cerebral softening, which was generally incomplete, distributed in the right orbital lobe, left parietal, left occipital and bilateral cingulate gyri. A group of big granulomas were found in the lower part of the left parietal lobe. The pathological changes in the right orbital lobe were similar to those of the chronic meningoencephalitis.

The pathological findings distinguished in this case were the features of the type of brain granulomas along with the type of meningoencephalitis. It was particularly noted that the widespread incomplete softening was observed in the white matter of the cerebrum following chronic schistosomiasis japonica.
