気脳術における断層撮影法の研究

STUDIES ON TOMOGRAPHY APPLIED TO PNEUMOENCEPHALOGRAPHY

　気脳術にさいし断層撮影を行つた70例につき，普通撮影の場合と比較して，その特徴を明らかにすることを目的とした。撮影方法としては，左右撮影の場合には正中矢状面を中心として1cm間隔に，また前後撮影の場合には中間質をとおる断面を中心として同じく1cm間隔に断層撮影を行つた。結果の主なものは次の通りである。

　1）断層撮影では，骨，洞をはじめ種々の不要な影が消去されるので，脳室の形態，輪郭などが明瞭にみられるようになる。

　2）正中矢状面における断層撮影では，中央脳室系（第3脳室，中脳水道，第4脳室），脳底部槽，後頭蓋窩（小脳），脳幹などを明らかにみることができる

　3）前後方向の断層像においては，側脳室，第3脳室，側頭葉領域などが明らかにされるが，中間質をとおる断面が最も大切である。

　4）上記の諸部位のうち，とくに普通撮影では出現しにくいものが明瞭にされることは，大きな利点である。

　5）検査すべき必要な脳の断面を随時えらぶことができ，また普通撮影にひきつづき行えるので被検者に新たな負担を加えなくてすむ。

　6）生体における脳のX線解剖学的見地からも価値がある。

The use of tomography with pneumoence-phalography in the diagnosis of neuropsych-iatric diseases was tried in 70 cases and the results were compared with those obtained by the routine method.

Method:

After the standard exposure were completed and the routine films examined, the patient was transferred to the table for the tomo-graphy. The head was positioned as for a anteroposterior roentogenogram and then was changed for a lateral one. A anteroposterior tomogram was made through the planes 7 to 13cm measured from the table top. The plane through the massa intermedia was the most important in anteroposterior projection and this plane was usually lied at 9 to 10cm from the table top. In several cases a pos-teroanterior tomogram was done.

Nextly, a lateral tomogram was made through the sagittal plane in midline and the plane 1cm above or under midline was also selected as desired. The lateral midline to-mogram and the tomogram through the ma-ssa intermedia are two standard tomographic levels.
