　【抄録】　Lerner Defense Scaleを用いて，女性17例の難治性単純肥満（SO）群と女性15例の健常（N）群のロールシャッハ・テストにみられるprimitive defense mechanisms（PDM）現象を比較し，人格構造の観点から，SOにおける統制困難な過食について検討した。PDMの比較では，SO群はN群に比して，borderline personality organization（BPO）における中心的な防衛メカニズムである分裂を有意に多く示した。また，投影性同一視は，統計的に有意ではないが，SO群のみに認められた。このようなPDMの布置は，SOにおける基礎をなすBPOを示すものである。そこで，基礎をなすBPOを共有するSOと摂食障害は，境界例症候群の1亜型と考えられた。SOは，女性に優勢であった。これは，体質的要因と環境的要因との相互作用による，女性に特徴的な攻撃性の抑制に基づくと考えられ，現代女性の行動化の変化と比較すると，SOにおける統制困難な過食は，古典的な防衛であると考えられた。

　Using the Lerner Defense Scale, the primitive defense mechanisms (PDM) observed in a Rorschach test in a severe simple obesity group (17 females) and in a normal control group (15 females) were compared. Then, from the stand-point of personality organization, hyperphagia in simple obesity was also investigated.

　The simple obesity group showed a significantly higher frequency of “splitting” of the PDM than the normal control group. “Projective identification” was noted only in the simple obe- Using the Lerner Defense Scale, the primitive defense mechanisms (PDM) observed in a Rors-sity group. The predominant constellation of PDM in severe simple obesity indicated the underlying borderline personality organization (BPO). These findings suggest that simple obesity and eating disorders, which hold the underlying BPOin common, belong to a subgroup of “borderline syndrome”.

　It is assumed that female predominance among simple obesity subjects is based on inhibition of aggression characteristic of the female by mutual correlation between constitutional and environmental factors. Futhermore, hyperphagia in simple obesity is thought to be closely associated with classical defense rather than with changes in recent female “acting-out”.
