 We report the case of a 62-year old right-handed man who developed a delusional misidentification of the environment following a subarachnoid hemorrhage and the surgical repair of a right middle cerebral artery aneurysm. After surgical treatment, he was transferred to Osaka Saiseikai Nakatsu Hospital in the center of Osaka City for the purpose of rehabilitation. While he quickly learned, and was able to remember, that he was at the Osaka Saiseikai Nakatsu Hospital, he insisted that the hospital was located in a huge replica of Osaka City, such as a movie set built in his rural home town. Here, we discuss false belief formation in detail, as was explained to the patient, from neuropsychological impairment to other psychodynamic factors.
