 Antipsychotics are often prescribed for delirium, however, the usage for the second-generation antipsychotics (SGA) has been increasing.

 Blonanserin (BNS) is SGA with a high affinity to a serotonin-5-HT2A-receptor and a dopamine-D2-receptor, which can treat hallucinations and delusions effectively. Additionally, it has low affinity for the receptors related to side effects, such as an adrenaline-α1, histamine-H1 and muscarinic acetylcholine-M1 receptors.

 We report the four cases of delirium treated effectively with BNS, and investigate the usefulness of BNS in delirium patients.
