我々は,10歳まで正常発達し,その後獲得されていた言語機能と下肢の運動機能の喪失が現れ,小児期崩壊性障害の診断基準を満たした14歳の女子症例を報告する。発症後次第に言語面での著しい退行,コミュニケーション能力の著しい低下およびきわめて強いこだわり,常同反復的な激しい顔叩きが出現した。2種類のセロトニン再取り込み阻害薬などを開始するも効果を認めず,aripiprazoleを24mgまで増量したところ顕著な改善を得た。このような状態はきわめてまれな事例であり,小児期崩壊性障害へのaripiprazoleの効果について初めての報告である。
We are presenting a case of a 14-year-old female child who presented with normal development until 10 years of age, followed by deterioration in previously acquired language skills and leg movements, suggestive of childhood disintegrative disorder. After the onset, significant regression in language gradually appeared, followed by significant reduction in communication skills and stereotypical repetitive violent face slapping. Subsequently, we administered two types of serotonin reuptake inhibitors, but there was no significant effect. Finally, after increasing the aripiprazole to 24mg, the patient experienced a significant improvement. We are reporting on this case becourse this paient's condition is very rare, and this is the first report on the efficacy of aripiprazole in a child with childhood disintegrative disorder.
