 Ulcerative colitis is an inflammatory disease of the large intestinal mucosa. The extent of mucosal inflammation is not always concordant with the clinical severity of the symptoms. It is thought that an exposure to psychosocial stressor and deflection of the character such as anxiety, obsession, irritation affects the clinical symptoms of the disease.

 We report the case of a patient with ulcerative colitis whose symptoms were remitted by cognitive therapy, and the good elapse continued. This case developed major depressive disorder after remission and exacerbation of the ulcerative colitis. Despite mucosal exacerbation, the clinical symptoms of the disease improved. This case suggests that cognitive therapy is effective in reducing the clinical symptoms of ulcerative colitis.

 Therefore, psychiatrists should play an active role in the treatment of ulcerative colitis.
