筆者がメンタルクリニックを開設した2005年8月から2年間に,新患として来院した病的賭博者100名(男性92名,女性8名)について臨床的実態を調べた。初診時年齢,ギャンブル開始と借金開始の平均年齢はそれぞれ,39.0歳,20.2歳,27.8歳であった。ギャンブルの対象は96名がパチンコ・スロットがらみで,初診までにギャンブルにつぎ込んだ平均金額は1,293万円,現在の平均負債額は595万円,28名が自己破産を含めたなんらかの債務整理を余儀なくされていた。17名にうつ病,5名にアルコール依存症が合併し,配偶者の15%がうつ病やパニック障害などで治療を受けていた。その他,婚姻状況・学歴・家族負因・受診経路・治療の概要などについても報告し,考察を加えた。
Clinical features of 100 pathological gamblers (92 men and 8 women) seeking treatment consecutively at the author's outpatient clinic during 2 years from August, 2005 were analyzed. Their actual average age, average age at gambling onset and average age at onset of borrowing money were 39.2, 20.2, and 27.8 years respectively. The most popular gambling forms were both pachinko and slot-machines (43%), slot-machines alone (22%) and pachinko alone (17%). Seven men were involved in illegal gambling. They had spent on gambling the amount of \12,930,000 on average and their current average financial debt was \5,950,000. Twenty-eight percent had been submitted legally for financial readjustments including bankruptcy. Their main psychiatric comorbidity was depression (17%) and alcohol dependence. Sixty-five% were married, 23% were single and 12% were divorced. Fifteen percent of their spouses were suffering from psychiatric disorders, mainly depression. One parent (father 24%, mother 5%) or both parents (7%) loved gambling. A high rate of pathological gambling, depression and alcohol dependence was found in their biological families. As for their educational status, 42% had a unversity degree. The overall results revealed that pathological gambling is a highly disabling disorder associated with disturbed marital life, financial problems and social difficulties. The need for their treatment and implications of prevention were also addressed in this paper.
