骨髄異形成症候群の患者で心筋壁内出血から心膜血腫を来して死亡した1剖検例

An Autopsy Case of Myelodysplastic Syndrome Death from Hemopericardium Associated with Myocardial Hemorrhage

要旨　患者は71歳，男性．骨髄異形成症候群，不応性貧血と診断されていた．汎血球減少に対して濃厚赤血球輸注および血小板輸注が行われていたが，抗血小板抗体が出現し，高度の血小板減少状態が続いていた．急激な胸内苦悶と呼吸困難，血圧の低下が出現し，死亡した．剖検にて心嚢内に400mlの血液が貯留しており，心タンポナーデが死因と考えられた．三尖弁付近の右心房に高度の壁内出血が認められ，同部から赤血球が漏出したものと考えられた．また，非出血部の心室壁にはリンパ球と形質細胞の集簇巣が認められ，心筋炎を伴っていた．本例のように高度の血小板減少症に加えて易感染性状態の患者ではウイルスなどの感染などをきっかけに急速に心タンポナーデを来して死亡する可能性があることが示唆された．

　We reported an autopsy case of myelodysplasticsyndrome with myocardial hemorrhage. A 71-year-old man who was diagnosed as having myelodysplasticsyndrome showed severe pancytopenia. He rapidlydeveloped dyspnea and hypotension, and death followed.At the postmortem examination, hemopericardium wasnoticed. We considered that cardiac tamponade was thecause of death. A bleeding tendency was seen in multipleorgans. It was noticed that extensive intramyocardialand epicardial hemorrhage had occurred at the rightatrium and ventricle. Though lymphocytes and plasmacells had infiltrated the left atrium and right ventricle, itwas suspected that the patient had developed myocarditis.
