IABP使用に際し側孔つきintroducerを用いた下肢虚血防止の工夫

A New Device:A fenestrated sheath for IABP, which reduces the incidence of ischemia in the lower limbs

　IABPのintroducerに側孔を開けたものを作製し，最も多く経験される下肢虚血の合併予防を試みた．同じIABP製品（TMP社製，9.5Fr）を用いた検討で，径1mmの38個の側孔を開けたintroducer（12 Fr）使用例45例と従来のシース使用例108例とで，合併症について比較検討した．従来のシース使用例では16.7％（18例）の頻度で下肢虚血の合併症が出現したのに対して，側孔つきintroducer使用例では身長，年齢，性別構成，IABP挿入時間に有意差がなかったにもかかわらず，下肢虚血の発生率が2.2％（1例）にとどまった（p＜0.005）．IABP挿入に際しガイドワイヤーが側孔にひっかかる点が欠点であったが，操作に注意すれば問題はなかった．側孔による血管壁やバルーンの損傷も経験されず，出血や血腫形成などの合併症もなかった．IABP挿入側の大腿動脈が細小な症例にも，下肢に血流が維持される側孔の工夫は下肢虚血合併症予防に有用であった．

To decrease the incidence of ischemia in the lower limbs, we developed a new introducer sheath for IABP, which had 38 side holes (diameter; 1mm).

Forty-five patients who received IABP counterpulsa-tion support with this fenestrated sheath were compar-ed with 108 patients in whom the ordinary nonfenestrat-ed sheath was used.

There was no significant statistical difference between these two study groups regarding age, height, weight and IABP insertion time.
