人工血管置換術施行の大動脈瘤11例を対象とし,術後に正常部である左総頸動脈,腹部大動脈,総腸骨動脈,外腸骨動脈,大腿動脈にてパルス・ドプラー法と電磁流量計を用い血流量を同時記録した.血管中軸部最高血流速度波形記録からのfiow velocity-time integralに,超音波断層像からの血管内径より求めた血管断面積を乗じてドプラー血流値を算出した.これは,同時計測の電磁流量計血流量と有意な正相関を認めた(電磁流量計血流量(ml・min−1)=0.587・ドプラー血流量(ml・min−1)+18.2,n=84,r=0.93,p<0.01).臨床例10例にてパルス・ドプラー法を用い,血流量を0.59・π(内腔半径[cm]2)・Integral[cm]・心拍数[bpm]とすると,左総頸動脈血流量251±62ml・min−1,腹腔動脈血流量608±399ml・min−1,上腸間膜動脈血流量687±381ml・min−1,腹部大動脈末端部血流量783±324ml・Min−1であり,局所血流値として妥当な値と考えられた.
A method of measuring volumetric flow in human arteries with pulsed Doppler technique was developed. Doppler flowmetry was assessed in 11 adult patients whose admission-diagnoses included aneurysm of the aorta or dissections. During surgery, regional blood flows of the left common carotid artery, terminal aorta, common iliac artery, external iliac artery or common femoral artery were measured by Doppler method and electromagnetic flowmeter. Eighty four arterial flow determinations were obtained from the 11 patients studied.
