　癌性心嚢炎により心タンポナーデをきたし心嚢ドレナージを行った16症例について，心嚢内薬物注入による心膜癒着術の有効性および問題点につき検討した．症例は肺癌が8例と最も多く，組織型は腺癌が81.3％を占めた．心嚢液は1例を除き血性で，腫瘍細胞陽性であった．心タンポナーデ発症後の平均生存期間は，32週であった．心嚢内薬物注入11例と未注入例5例の2群間で生存率の比較を行い，各群の中間生存期間は16週，19週と有意差はなかった．再貯留は注入群3例（18％），未注入群3例（60％）で，未注入群で多い傾向にあった．再貯留例には全例アドリアマイシンが注入されていた．マイトマイシンC注入4例，シスプラチン1例に再貯留はなかった．また，薬物注入による重篤な副作用もみられなかった．

　心嚢内薬物注入による心膜癒着術は延命効果はなかったが，再貯留予防に効果があり，本症に対する有効な治療法であると思われた．

We studied the effects of local therapy on cardiac tamponade caused by carcinomatous pericarditis in 16 cases.

Five out of 16 cases (drainage group) received treat-ment by only drainage of pericardial effusion, and the remaining 11 cases (infusion group) had local infusion of antineoplastic agents into the pericardium after drainage. The median survival time was 16 weeks in the infusion group and 19 weeks in the drainage group. There was no statistically significant difference. Reac-cumulation of pericardial effusion was recognized in 3 patients in the drainage group (60%), and in 3 patients in the infusion group (18%).
