てんかんを伴った大動脈炎症候群1症例の携帯型24時間ポリグラフ法による検討

Analysis of EEG, respiration, and ECG by 24-hour polygraphic study in a case of aortitis syndrome

　大動脈炎症候群は，全身の動脈系に原因不明の非特異的慢性炎症による閉塞，拡張を来たし全身臓器に多彩な臨床症状を呈する。今回我々は，てんかんを伴い大動脈弁閉鎖不全と弁上大動脈瘤を呈する患者に，携帯型24時間ポリグラフ法を施行した。携帯型24時間心電図は，不整脈や虚血性心疾患において無症候性の場合にも診断・治療・予後推測に有用であると知られている^1）。我々はさらに，呼吸，脳波を同時記録することにより，生命中枢である心血管—呼吸—脳相関を知ることが，特に全身疾患の病態解析において重要と考える。日常生活中のポリグラフ解析は報告されていない。

　本症例で，無症候性欠神てんかん発作波，無呼吸発作，睡眠時呼吸—心拍異常反応が観察された。これらは自覚症状や生体活動と相関し，治療により改善し，多彩な全身疾患の治療・予後推測に有用であったので報告する。

The relationships betweeen electroencephalogra-phic changes and cardio-respiratory responses were analysed by using 24 hour ambulatory simultaneous monitoring of EEG, respiration, ECG in a patient with aortitis syndrome. Circadian changes of "ab-sence" attacks, effects of anti-convulsive drugs on epileptic attacks, and respiratory arrest and abnor-malities of the cardiorespiratory integration were clarified.
