  • Look Inside

Chylopericardium is a rare disease and its cause is unclear in many case. We have experienced a case of chylopericardium to be caused by thrombus of superior vena cava, which gave us a chance to learn its underlying cause. A 28-year old man was admittid because of dyspnea on exertion and edema. Cardiac enlargement and bi-lateral pleural effusion were noted. Large quantities of pericardial effusion were also detected by echocardiography. Pericar-diocentesis showed chyle and the patient was diag-nosed as having chylopericardium. In addition, the CT scan of the thorax revealed thrombus formation in the superior vena cava. Occlusion of the subclavian veins was recognizes by venography. These fiin-dings, suggested occlusion at the angle of union of the internal jugular and subclavian veins as a cause for chylopericardium. The patient also presented nodular erythematous eczema and thrombophlebitis, and showed positive response to a needle, etc. Thus Behcet disease was taken into account as an underly-ing disease. Abnormality in the lymph duct system in Behcet disease is known to be rare. A case com-plicated with chylopericardium has not been repor-ted so far. The patient is now under steroid treat-ment with a satisfactory result.
