僧帽弁逸脱，僧帽弁閉鎖不全および高度三尖弁閉鎖不全を合併した高齢者心房中隔欠損症—その適応と術式の選択に関する考察

　各種の器質的心疾患に房室弁逆流が合併する頻度は決して少なくないことが注目されている。その際，主病変の外科治療時に何らかの処置を加えるべきかどうか，またどのような術式をとるべきか苦慮する場合は稀ではない。

　今回，著者らは僧帽弁逸脱（MVP）による僧帽弁閉鎖不全（MR）および弁輪拡大による高度三尖弁閉鎖不全（TR）を合併した高齢者心房中隔欠損症（ASD）に開心術を施行し，良好な成績を得たので，その適応，術式の選択に関し若干の考察を加え報告する。

A 61-year-old man with atrial septal defect (ASD) combined with mitral regurgitation (MR) due to mitral valve prolapse and sever tricuspid regurgita-tion (TR) was reported.

Two-dimensional Doppler Color Flow Mapping Echocardiography (2-DD) was used for assessment of the grade of MR and TR and the indication for MR and TR.

2-DD findings of the minimal MR and sever TR signals led to Triuspid annuloplasty (=De Vega's maneuver) and no surgical intervention for mitral valves associated with ASD closure, which led togood postoperative course.

2-DD was thought to be useful to determine the procedure for atrio ventricular valvular regurgita-tion associated with other major cardiac lesion.
