Eight patients with paroxysmal supraventricular tachycardia (PSVT) underwent serial electrophysiologic studies using transesophageal pacing (EPS-EP) to deter-mine the predictive value of the EPS-EP in the selec-tion of antiarrhythmic therapy and to establish the therapeutic efficacy of available agents.

In each patient PSVT could be reproducibly initiated by burst or programmed stimulation. After control EPS-EP, the effects of serial drugs (diltiazem, disopy-ramide, propranolol, digoxin, procaine amide) on the ability to initiate PSVT or the alteration to tachycardia zone. Mechanisms of PSVT were also examed using transesophageal electrocardiogram and EPS-EP.

Drug responses are variable. This study suggests that serial EPS-EP provides rapid identification of successful antiarrhythmic therapy and may predict in which patients conventional therapy would be ineffective.
