Ortner症候群(以下O症と略)は,別名"cardio-vocalsyndrome"とも呼ばれ,内因性の心疾患に伴なつて生じる左側声帯麻痺に与えられた名称である1)。本症候群は僧帽弁狭窄(以下MSと略)によって起こることがよく知られているが2,3),原発性肺高血圧(以下PPHと略)による報告は少ない4)。
今回我々はPPHに伴なうO症の1例を経験したので報告し,本症候群における左側声帯麻痺の発生機序について考察を加える。
The "Ortner's syndrome" is originally a term which has been used in reference to those cases of left recurrent laryngeal nerve paralysis related to intrinsic heart disease. A case of Ortner's syndrome associated with primary pulmonary hypertension (PPH) was presented.
A 34-year-old female was admitted with chief complaints of hoarseness and exertional dyspnea. Physical and various clinical examinations including cardiac catheterization verified the diagnosis of PPH. By visualization of the larynx, left vocal cord paral-ysis due to left recurrent laryngeal nerve paralysis was observed. Therefore, left recurrent laryngealnerve paralysis in this case was discussed, and thediagnostic importance of underlying disease wasstressed.
