The "Ortner's syndrome" is originally a term which has been used in reference to those cases of left recurrent laryngeal nerve paralysis related to intrinsic heart disease. A case of Ortner's syndrome associated with primary pulmonary hypertension (PPH) was presented.

A 34-year-old female was admitted with chief complaints of hoarseness and exertional dyspnea. Physical and various clinical examinations including cardiac catheterization verified the diagnosis of PPH. By visualization of the larynx, left vocal cord paral-ysis due to left recurrent laryngeal nerve paralysis was observed. Therefore, left recurrent laryngealnerve paralysis in this case was discussed, and thediagnostic importance of underlying disease wasstressed.
