メサコリンおよびヒスタミン吸入試験に異常が認められなかったダニ・アレルギー性喘息の1例

A case of mite sensitive asthma without methacholine and histamine hyperresponsiveness

　アメリカ胸部疾患学会の気管支喘息の定義には気道過敏性という概念が含まれている^1）。

　気道過敏性とは健康人には悪影響を及ぼさないような弱い刺激に対して過剰に反応して，気道閉塞が発現する病態と理解されている。そして臨床的にはメサコリンやヒスタミンなどの薬物による吸入試験や，運動誘発試験などによって気道過敏性の存在や程度を把握することができる。小児の場合にはアレルギー性喘息が多いため，アレルギーに関する検討が中心に行われており，気道過敏性に関する検討は比較的少ない。しかし，アレルギー素因が存在していても気道過敏性が存在しなければ喘息としては発症せず，アレルギー性鼻炎やアレルギー性結膜炎，アトピー性皮膚炎などとして発症するものと考えられている。すなわち，小児においてもアレルギーと気道過敏性の両面からの検討が必要である。

We repored a S years old boy with bronchial asthma. He had the episode of typical asthmatic attacks for about 2 years with high recurrency. He showed the sensitivity against house dust mite (Dermatopha-goides farinae, Df) by the skin test and IgE radio-allergosorbent test (IgE-RAST). In the inhalation test using Df extract, isolated late asthmatic response was observed without immediate asthmatic response. Fur-thermore, bronchial hyper-responsiveness was not dete-cted by both methacholine and histamine inhalation challange. These data suggested that mite induced late asthmatic response could be induced in patient without methacholine and histamine hyperresponsiveness.
