下肺静脈のvaricosityを伴ったScimitar症候群の1例

A case of scimitar syndrome associated with varicosity of pulmonary vein

　Scimitar症候群は，部分肺静脈還流異常の一型で，1960年Neilら^1）が，「右肺静脈の下大静脈への異常還流」「右肺低形成」「心臓の右方転位」を三徴とする症候群に命名した比較的まれな疾患である。scimitarとは片刃の彎曲したサーベルのことで，下大静脈への異常静脈が胸部X線上，形態的にこれと類似するためscimitarsignと名付けられた^2）。われわれは胸部X線写真でscimitar signを認め，本症を疑って精査したところ，下肺静脈のvaricosity （静脈瘤様拡張）を伴った典型的Scimitar症候群と診断した1例を経験したので，若干の文献的考察を加えて報告する。

A case of Scimitar syndrome was reported. A 53-year-old man was found scimitar sign on a routine chest rentogenogram. Further examination demon-strated varicose changes of the right lower pulmo-nary vein, hypoplasia of the right lung and pulmo-nary artery, an anomaly of the right lower lobe bronchus, and cardiac displacement to the right in addition to anomalous return of the right upper pulmonary vein to the inferior vena cava.

A brief review of the literature was given.
