To assess the effect of nifedipine on left ventri-cular function in patients with dilated cardiomyo-pathy, echocardiography at rest and during supine bicycle exercise was performed in 10 patients before and after the sublingual administration of nifedi-pine 10mg.

There was no significant change in resting indices of left ventricular performance before and after the administration of nifedipine. However, during exer-cise nifedipine lowered total peripheral resistance index from 2016+851 to 1362+584 dynes・sec・cm-5・M2 (p<0.05). Simultaneously all indices of left ventricular performance during exercise improv-ed : per cent fractional shortening rose from 16.5 ±10.3 to 20.3±8.3 per cent (p<0.05), mean velocity of LV posterior wall in systole from 24.0±8.9 to 33.4±9.0mm/sec (p<0.01), ejection frac-tion from 32.6±17.9 to 40.2±13.7 per cent (p<0.05), and cardiac index increased from 3.84±1.55 to 4.78±1.07 L/min/M2 (p<0.05). Left ven-tricular end-diastolic dimension during exercise di-minished after the administration of nifedipine from 63.4±5.0 to 61.1±5.2mm (p<0.05). But, no significant changes in heart rate were noted both at rest and during exercise.

These data suggest that nifedipine improved car-diac reserve in patients with dilated cardiomyo-pathy by reducing afterload during exercise.
