  • Look Inside

A case of severe disopyramide poisoning with the aortic valvular regurgitation was treated success-fully. A 36-year-old male was admitted to hospital four hours after ingesting a large dose of disopyra-mide. On arrival he was conscious but BP was 86/46 mmHg. The ECG showed sinus tachycardia with long QT interval, wide QRS duration and LBBB pattern. He suddenly lost consciousness and BP was unrecordable. The ECG showed VT and VF. Iso-prenaline was given and transient pacemaker was inserted. Atypicel VT was repeated and haemody-alisis with charcoal filter was started to remove disopyramide. After 2 hour-HD, he became alert, and the ECG showed narrowing of QRS complexes with the increasing of BP. He recovered completely but he underwent an AVR after three months.
