完全房室ブロックにおける一方向性ブロックの表現としてのretrograde concealed conduction

Retrograde concealed conduction as a manifestation of unidirectional block in complete A-V block

　完全房室ブロックは，房室伝導系において順方向への興奮伝達が完全に途絶した状態で，房室結節，His束内あるいはHis束以下の伝導系の障害により生じる。しかし，完全房室ブロック例でも室房伝導が可能な例（一方向性ブロック）があることが指摘され，近年，種々の不整脈例における室房伝導が注目され，いくつかの報告が散見される^1〜4）。

　今回，著者らはHis束電位図法により，His束内あるいはHis束以下のブロックと診断した完全房室ブロック例における房室結節部へのretrograde concealed con—ductionについて検討した。

Retrograde concealed conduction was investigated electrophysiologically in 24 cases with complete A-V block who did not show manifest evidence of V-A conduction.

When a sinus P wave occurred soon after the ven-tricular complex, the prolongation of A-H interval was observed in 4 of 24 cases (16.7%). This phe-nomenon was considered to be due to retrograde concealed conduction of ventricular impulse to the A-V node and a manifestation of unidirectional block in A-V conduction system.

We postulated that the incidence of the unidirec-tional block in complete A-V block might be much higher than that reported previously.
