異型狭心症発作時の標準12誘導心電図にて房室結節二重経路の存在が示唆された1例

A case whose surface electrocardiogram suggested the evidence of dual A-V nodal pathways during the attack of variant angina

　房室結節二重経路（DP）は，最初Rosenら^1）（1972）により報告されたが，その後の研究により稀な現象でないことが明らかとなった^2〜5）。しかし，標準12誘導心電図所見からその存在を知り得た例の報告は少ない^4,6,7）。

　著者らは，異型狭心症発作時の標準12誘導心電図において，著明な洞徐脈とともに2種のPR間隔を示し，DPの存在が疑われ，電気生理学的検査によりその存在を証明し得た1例を経験した，本例においてはDamato分類^8）IV型のgap現象が認められ，その出現機序についても考察を加えた。

A case whose conventional ECG showed two kinds of PR intervals during the attack of variant angina was reported. In electrophysiological study, the atrial extrastimulus technique revealed dis-continuous A-V conduction curve ( jumping phenomenon ) indicating dual A-V nodal pathways. Moreover, A-V nodal gap phenomenon ( Type 4 of Damato's classification ) was observed in this case. Initial site of block was present within a distal portion of the A-V node and the conduction delay occurred within the slow pathway of the A-V node. The mechanism of this conduction delay was considered due to the dual A-V nodal pathways.
