A case whose conventional ECG showed two kinds of PR intervals during the attack of variant angina was reported. In electrophysiological study, the atrial extrastimulus technique revealed dis-continuous A-V conduction curve ( jumping phenomenon ) indicating dual A-V nodal pathways. Moreover, A-V nodal gap phenomenon ( Type 4 of Damato's classification ) was observed in this case. Initial site of block was present within a distal portion of the A-V node and the conduction delay occurred within the slow pathway of the A-V node. The mechanism of this conduction delay was considered due to the dual A-V nodal pathways.
