心筋梗塞の合併を思わせた肥大型閉塞性心筋症—心電図異常Q波に関する考察

A case of hypertrophic obstructive cardiornyopathy simulating myocardial infarction:With a special reference to abnormal Q-waves on electrocardiogram

　心エコー図法，心機図法などの進歩に伴って，肥大型閉塞性心筋症hypertrophic obstructive cardiomyopathy，HOCM　（一名特発性肥厚性大動脈弁下狭窄idiopathichypertrophic subaortic stenosis，IHSS）が診断される機会が多くなった^1〜3）。

　私どもも過去5年間に15例の本症を経験し，65歳以上の高齢者も3例を数えたので最近報告している^4）。

A 58-year-old male patient hospitalized with extensive abnormal Q-wave, atrial fibrillation and congestive heart failure finally diagnosed as an advanced stage of a hypertrophic obstructive cardiomyopathy was demonstrated.

Although complication of infero-anterior myo-cardial infarction could not be ruled out because of electrocardiographic and scintigraphic abnor-malities, infarction caused by extramural coronary occlusion was ruled out by normal coronary arteriographic patterns.
