犬冠動脈前中隔枝結紮による実験的His束内ブロック

Experimental intra-His bundle block following ligation of the anterior septal artery in dogs

　1969年カテーテル電極を用いたHis束心電図の記録法^1）が発表されて以来，従来のスカラー心電図によるブロックの部位診断に加えて，房室ブロックがHis束の上下どちらで生じているかまでが診断できるようになった。

　His束心電図法の普及の結果，ヒトにおいてHis束内ブロックがかなりの頻度でみられることが明らかとなったが，その進展様式を観察することは困難である。今回われわれは犬の冠動脈前中隔枝を結紮して実験的にHis束内ブロックを作製し，その進展様式，II°His束内ブロックの自然発生に先行して心房ペーシングによりII°His束内ブロックの発生する条件，His束内ブロックにおけるWenckebach周期などについて検討した。

The present investigation was performed in order to study the pathophysiology of intra-His bundle block (IHBB) following the ligation of the anterior septal artery in the anesthetized dogs. The His bundle potential was recorded by the electrode catheters and bipolar plunge wire electrode. Validation of the split His potential was made by atrial pacing, vagal stimulation and His bundle pacing.

An IHBB of various degrees developed following the ligation of the anterior septal artery in 13 of 15 cases.
