活動期感染性心内膜炎に合併する四肢末梢血管の感染性動脈瘤

Peripheral Mycotic Aneurysm in Infective Endocarditis

要旨　感染性心内膜炎(IE)に合併する四肢末梢血管の感染性動脈瘤の発生は稀である．2004年12月～2008年12月までの期間に行われた感染性心内膜炎に対する心臓手術症例26例中で，四肢末梢血管の感染性動脈瘤を合併した症例は2例であった．1例目は65歳男性，僧帽弁位に発生した活動性IEで右上腕動脈に感染性動脈瘤を発症した．感染性動脈瘤を切除した後に大伏在静脈による血行再建術が行われ，4週間の抗生剤投与の後に僧帽弁置換術が行われた．2例目は37歳男性で，僧帽弁位の感染性心内膜炎で僧帽弁形成術が行われた14日後に右尺骨動脈に感染性動脈瘤を発症した．感染性動脈瘤が切除された後に，端々吻合による切除断端の血行再建が行われた．2例とも術後IEの再燃は認められず，経過良好である．

　Peripheral mycotic aneurysm is a rare complication in infective endocarditis(IE). Twenty-six patients underwent cardiac operations for IE between December 2004 and January 2008 at our institute. Among these, two patients had peripheral mycotic aneurysm (the right brachial artery and right ulnar artery). One patient underwent aneurysm excision preoperatively, and the other patient postoperatively. Vascular reconstruction was performed using saphenous vein. At follow-up, both patients are doing well without recurrence of infection.
