食道ESDによる瘢痕近傍の表在性扁平上皮癌の病理組織像

Histopathology of Superficial Squamous Cell Carcinoma near the Scar Following Esophageal Endoscopic Submucosal Dissection

要旨●食道癌に対して内視鏡的粘膜下層剝離術（ESD）が施行された後の近傍に食道癌が発見されることがある．ESD治療の影響による組織修飾が加わっているため，粘膜固有層から粘膜下層にかけて線維化を呈する．病理組織学的には粘膜筋板が不明瞭になるために深達度の評価に注意する必要があるが，実際のところこのような症例についてはスクリーニングが十分に行われているため，早期の表在性扁平上皮癌で見つかることが多く，粘膜筋板の走行に注意を要する場合は少ない．食道癌に対するESD後に発見された瘢痕近傍病変について，内視鏡検査での範囲診断が困難であった症例の病理組織像を提示する．

　After ESD（endoscopic submucosal dissection）for ESCC（esophageal squamous cell carcinoma）, a new esophageal squamous cell carcinoma may occur near the site of scar formation. Fibrosis is observed from the lamina propria to the submucosa due to tissue modification by the effects of ESD treatment. Histopathologically, the muscularis mucosae becomes obscured, so it is necessary to pay attention to the evaluation of the invasive depth. However, since such patients are well screened, so secondary squamous cell carcinomas are often found at early-stage as a superficial type, the running of muscularis mucosae rarely require attention for pathological diagnosis. We present the patients in which endoscopic assessment of lesions near the scar after ESD for ESCC was problematic.
