要旨●大腸鋸歯状病変のうちSSA/PはWHO分類(2010年)でSSA/P with cytological dysplasiaとして悪性化ポテンシャルを有することが明記された.その後,WHO分類(2019年)ではSSA/Pの名称はなくなり,代わりにSSLという名称が加わった.WHO分類(2019年)では名称だけではなく,病理組織学的診断基準も旧分類と若干異なり,臨床的な取り扱いはもちろん病理組織学的な取り扱いについても混沌としている.今回,当センターで経過観察しえたSSA/P関連病変を提示し,SSA/Pの癌への発育進展について考察した.経過観察中に変化を来さない病変が存在する一方で,段階的な変化を認め癌の診断となった病変が存在した.変化を来した症例ではdysplasiaの特徴とされる発赤や二段隆起に先駆けてpit patternの変化を認識できる可能性があり,SSA/Pの発育進展を内視鏡で捉えるためには,拡大内視鏡による詳細かつ丁寧な観察を行う必要性があると考えられた.
SSA/P(sessile serrated adenoma/polyp)was specified as having malignant potential as SSA/P with cytological dysplasia by the WHO classification of tumours in 2010. Subsequently, in the WHO classification of tumours in 2019, the name SSA/P was eliminated and SSL was added instead. In the new classification, the histopathological diagnostic criteria also differed from the old classification, resulting in chaos in terms of clinical, as well as histopathological treatment. This study presented the SSA/P-related lesions observed at our center and discussed the progression of SSA/P into cancer. While some lesions did not change during follow-up, others showed progressive changes that led to a cancer diagnosis. In the cases with changes, it was possible to recognize the changes in the pit pattern prior to erythema and double-stage elevation, which are considered to be dysplastic characteristics. Therefore, it was considered necessary to perform detailed and careful observation using a magnifying endoscope in order to capture SSA/P's developmental progress endoscopically.
