要旨●明らかな転移を伴わない粘膜下層への浸潤が疑われるcT1b食道扁平上皮癌に対する内視鏡的切除＋化学放射線療法（CRT）の有用性が報告され，今後治療選択の一つとなりうる可能性が示唆されている．当院においても良好な結果であり，現在の食道癌診療ガイドラインで推奨されている治療との比較，断端陰性で切除するための診断・技術的な課題など，さらなる検討が必要ではあるが，粘膜下層への浸潤が疑われるcT1bに対する内視鏡治療＋CRTは侵襲性，臓器温存性の点からも妥当性があると考えられる．

　Endoscopic resection followed by CRT（chemoradiotherapy）was found to be effective for cT1bN0M0 ESCC（esophageal squamous cell carcinoma）, suggesting that this may be one of the treatment options in the future. Although more research is needed to compare the results of endoscopic resection with those recommended in current guidelines, as well as to address diagnostic and technical issues in resection with negative margins, the clinical outcome of endoscopic resection followed by CRT for cT1b ESCC in our hospital was also good, and this treatment may be feasible in terms of invasiveness and organ preservation.
