Prognosis and Recurrence of T1 Colorectal Carcinoma

要旨●大腸T1癌は，病理学的リンパ節転移high risk症例と直腸病変で再発率が高い．特に，内視鏡切除のみで経過観察された病理学的リンパ節転移high risk症例は再発率が高いので，症例に応じて適切な追加治療を行う必要がある．また，同時性遠隔転移の頻度は低いが，いったん遠隔転移を起こすと予後不良となるため，T1癌を診断したときには全身を精査しておく必要がある．

　The recurrence rates of T1 colorectal carcinoma in high-risk pathological cases for lymph node metastasis and lower rectal lesions are high. Additionally, the pathological high-risk cases for lymph node metastasis resected by endoscopy and followed up without additional treatment may show a high recurrence rate ; therefore, appropriate additional treatment should be performed based on each case condition. Although the synchronous distant metastasis of T1 colorectal carcinoma is very rare, it is reported to show a very poor prognosis. It is recommended that blood examination, including that of tumor markers such as CEA, and whole-body computed tomography scan are performed before the treatment of T1 colorectal carcinoma.
