要旨●欧米ばかりでなく本邦においてもBarrett食道腺癌の増加が報告されているが，表在型Barrett食道腺癌に対する内視鏡治療後の予後が良好なことも明らかになっている．したがって，Barrett食道腺癌をいかに早期に発見できるか，すなわちBarrett食道腺癌のsurveillanceが重要となっている．欧米では，多くのガイドラインで2〜5年に1度の内視鏡検査によるsurveillanceが推奨されているが，本邦ではsurveillanceの方法が確立していないのが現状である．しかしながら，本邦でもLSBEからの発癌率は欧米からの報告と同程度であることが報告されており，LSBEはsurveillanceの対象になると考えられる．今後，本邦におけるBarrett食道腺癌のsurveillance法の確立が望まれる．

　The incidence of Barrett's esophageal adenocarcinoma has increased not only in the Western countries but also in Japan. The prognosis after endoscopic treatment for superficial Barrett's esophageal adenocarcinoma has been reported to be good. Therefore, the surveillance of Barrett's esophageal adenocarcinoma is important to detect cancer at an earlier and potentially curable stage. In the Western countries, some guidelines recommend endoscopic surveillance once every two to five years, but no surveillance strategy has been proposed in the official Japanese guideline. However, it has been reported in Japan that the novel adenocarcinoma's incidence rate in patients with LSBE（long-segment Barrett's esophagus）is similar to the values reported in the Western countries. Thus, surveillance for LSBE is needed in Japan. Hopefully, a surveillance strategy for Barrett's esophageal adenocarcinoma will be established in the near future in Japan.
