知っておきたい直腸肛門部の腫瘍性疾患

Differential Diagnosis of Malignant Lesions in the Anorectal Region

要旨●肛門周囲・直腸肛門部には，日常診療でよく経験するいわゆる肛門疾患以外に，種々の悪性腫瘍，口側からの脱出性病変，非上皮性腫瘍，炎症性腸疾患の肛門病変，sexually transmitted diseaseを含めた感染性疾患，薬剤起因性疾患など極めて多彩な疾患がみられる．これらの多彩な疾患の中で増加する悪性腫瘍を見逃さないことは直腸肛門部疾患診療において極めて重要である．本稿では多彩な疾患の中で直腸肛門部において“目で見ることができる病変”，“怒責状態で見ることができる病変”，“内視鏡で視ることができる病変”に注目し自験例を供覧，その鑑別診断・治療について報告する．そしてこれらの症例を通じて“肛門を見る・視る，そして診ること”の重要性を強調したい．

　Perianal and anorectal lesions can indicate various diseases. Apart from common benign anal disease, there are many kinds of malignant tumors, prolapsed disease, nonepithelial tumors, anal disease due to inflammatory bowel disease, and infectious disease including sexually transmitted disease found in the anorectal region. Thus, it is extremely important to not miss malignancies in the anorectal region among various diseases in clinical practice.

　In this article, we focus on anorectal lesions diagnosed using straining diagnosis or endoscopy and report the diagnosis and treatment of our cases. Moreover, we emphasize the importance of careful observation of perianal lesions using whole method.
