─早期胃癌の時代的変遷─臨床病理学的変遷

Changing trends of Clinicopathological Findings of Early Gastric Cancer

要旨●1991〜1993年までの163例180病変，2003〜2005年までの249例279病変，そして2015〜2017年までの400例442病変の早期胃癌を比較したところ，①早期胃癌治療対象者の平均年齢は高齢化がみられた，②比較的小さな粘膜内癌，すなわち初期病変が増加していた，③肉眼型では0-IIbを伴う病変が増えており，0-IIIを合併した病変が減少していた，④組織型は低異型度分化型癌（超高分化腺癌）が増加している一方で，未分化型癌の割合が減少していた．

　We studied specimens of early gastric cancer obtained from surgical and endoscopic resections from three periods：1991-1993, 2003-2005, and 2015-2017, and investigated the changes in the trends of clinicopathological findings. We found an increase in the age of patients with early gastric cancer, and in the number of lesions in early stage among treatment-target lesions. Macroscopically, the number of lesions presenting IIb morphology was observed to have increased while the number of ulcerated lesions（type III）had decreased. Histologically, the number of low-grade well-differentiated adenocarcinomas increased, while the percentage of poorly-differentiated adenocarcinomas and signet-ring cell carcinomas decreased.
