好酸球性食道炎の食道運動障害

Esophageal Motility Disorders in Patients with Eosinophilic Esophagitis

要旨●好酸球性食道炎では高率に食道運動障害が認められることが報告されている．好酸球性食道炎に伴う食道運動障害は，さまざまなタイプが認められることが特徴であり，食道アカラシアと同様の食道内圧所見を呈する症例もある．近年，食道の伸展性を評価することができるendoluminal functional lumen imaging probe（Endoflip^®）が開発され，好酸球性食道炎では食道の伸展性が低下していることが明らかになった．また，超音波検査を食道内圧検査に併用した検討では，好酸球性食道炎では食道の輪状筋と縦走筋の協調性が低下している可能性も指摘されている．治療前後の食道内圧所見をみると，好酸球性食道炎に対する治療において多くの症例で食道運動障害が改善することが報告されている．

　Esophageal motility abnormalities are often seen in patients with EoE（eosinophilic esophagitis）. Patients with EoE may have various types of esophageal motility abnormalities. For example, esophageal manometry showed esophageal achalasia in a patient with EoE, which improved after corticosteroid administration. Recently, an endoluminal functional lumen imaging probe（Endoflip^®）, which can assess esophageal distensibility, has been developed. A study using Endoflip^® showed a decrease in esophageal distensibility in patients with EoE. In addition, synchronous movement of esophageal inner and outer muscles is impaired in patients with EoE, which was revealed using a combination of ultrasonography and esophageal manometry. Esophageal motility abnormalities may be improved by treatments for EoE.
