潰瘍性大腸炎の術後に呼吸器病変，胃・十二指腸病変を合併した1例

Bronchopulmonary and Gastroduodenal Lesions Developing-Colectomy for Ulcerative Colitis, Report of a Case

要旨　患者は65歳，女性．2007年に潰瘍性大腸炎（UC）を発症した．下痢や血便などの症状が増悪したため，2011年10月中旬に入院となった．薬物治療抵抗性であったため，11月下旬に結腸亜全摘術＋回腸人工肛門造設術を施行した．術後，プレドニゾロン（PSL）の中止に伴って，気管支・肺病変を発症したが，PSLで軽快した．しかし，PSLの中止に伴い，新たに胃・十二指腸病変と回腸囊炎を合併した．PSLの投与により，気管支・肺病変と胃・十二指腸病変は寛解したが，回腸囊炎は改善しなかった．今回，UCの術後に複数の合併症を発症したまれな症例を経験したので報告する．

　We report a case of plural complications that developed after colectomy of UC（ulcerative colitis）. 65-year-old female with a 4-year history of UC was hospitalized in October 2011 because of diarrhea and bloody stools. Because her symptoms were refractory to medical treatments, colectomy was performed in late November. After the colectomy, prednisolone was with drawn, following which she developed bronchopulmonary and gastroduodenal lesions, as well as ileal pouchitis. PSL was recommenced, which resolved both the bronchopulmonary and gastroduodenal lesions, but not the pouchitis.
