Magnified Endoscopic Classification for Superficial Esophageal Squamous Cell Carcinoma: Classification of the Japan Esophageal Society

要旨　日本食道学会では，一般会員が使用しやすい，より簡略化した拡大内視鏡分類を提唱した．本分類では食道扁平上皮における基本的な血管構造であるIPCLに注目し，異常血管がループを保っている場合をB1，ループが消失している場合をB2，B2の3倍以上（60μm以上）の太い異常血管をB3と表記し，深達度診断へ応用した．また，無血管野（avascular area）をsmall，middle，largeへ亜分類し，深達度診断へ応用した．本稿では日本食道学会分類の基本を述べ，今後の課題に関して言及した．

　The Japan Esophageal Association has established a new simple classification for superficial esophageal squamous cell carcinoma in order to diagnose the depth of invasion of tumors. The classification is based on the presence of loop formation and thick abnormal vessels and includes the following categories : B1, presence of a loop-like shape ; B2, absence of a loop-like shape ; and B3, presence of very thick（60μm or thicker）vessels. This study describes the effectiveness and limitations of this new classification.
