 The diverticular disease of the colon is classified into four types by Perry & Morson: 1) diverticulosis of the sigmoid colon with muscle abnormality, 2) diverticulosis of a part or the whole colon without muscle abnormality, 3) diverticulum of the cecum or ascending colon, 4) right-sided diverticulosis. Rightsided diverticulosis is rare in Caucasians but is the ccmmcnest type in Japan. In an attempt to clarify the etiology of right-sided diverticulosis, an in-depth histopathological study was conducted, and microangiography was performed on the excised segment of right-sided diverticulosis. The results indicated that most diverticula of right-sided diverticulosis were the so-called false one, with a close relation between diverticula and vessels, and many of cases of right-sided diverticulosis, like those occuring in the left segments, had characteristic muscle abnormalities, i. e., shortening of the colon and thickening of tenia and inner circular musculature. Thickening of tenia was found in eight of twelve cases of right-sided diverticulosis (67%), and thickening of inner circular musculature was found in 12 of 15 (80%) also. All 8 cases of right-sided diverticulosis which had thickening of tenia were accompanied by thickening of inner circular musculature. But the etiology of right-sided diverticulosis of the colon is still unknown. In the future we think that it is necessary to measure the intracolonic pressure of right colon by the use of colonofiberscope.
