十二指腸乳頭下部潰瘍の1例

The Postbulbar Duodenal Ulcer beyond the Papilla; A Report of a Case

　十二指腸潰瘍はX線診断上その95％が球部に発生するといわれ，病理学的にも幽門輪に近いほど発生頻度が高い．球部以外に発生する十二指腸潰瘍はまれなものと考えられる．球部より肛側の十二指腸潰瘍は球部潰瘍とやや病像を異にするので一括して十二指腸球後部潰瘍Postbulbar duodenal ulcerの呼称で検討されている．Meshanは十二指腸潰瘍を発生部位によりbulbar，postbulbar，distalの三者に分類し，その頻度はbulbar95％，postbulbar 4％ distal 1％で，遠位の十二指腸潰瘍が非常にすくないことを示している．

　筆者らは上腹部痛を主訴として来院した26歳の男性にみられた十二指腸乳頭下部潰瘍の比較的まれな症例を経験したので，若干の文献的考察を加えて報告する．

　A twenty-six-year-old male was admitted with chief complaints of stasis of the meal and a dull pain in the right upper quadrant of the abdomen. Routine X-rays of the upper gastrointestinal tract showed a niche, convergence of mucosal folds, and a narrowing of the duodenal lumen at the second portion of the duodenum. Detailed survey of the duodenum by the hypotonic duodenography revealed an ulcer at the second portion of the duodenum, lying distal to the papilla. At the operation, an ulcer was found at the antero-medial of the duodenum, slightly anal to the ampulla of Vater with a marked periserosal adhesion to the head of the pancreas.

　Rarity of the incidence of duodenal ulcer in the distal segment has been discussed.
