わずかな伸展不良で発見された微小純粋Ⅱb型早期胃癌の1例

Small Genuine Ⅱb Type Carcinoma of the Stomach Presenting Poor Gastric Wall Stretching as an Only Clue to the Diagnosis, Report of a Case

はじめに

　Ⅱbに関しては1971年第13回日本消化器内視鏡学会で初めて取り上げられ，われわれも随伴Ⅱbを対象に，Ⅱb・類似Ⅱbの摘出標本レントゲノグラムを検討し，そのX線像を検討した．その後，術前に発見されたⅡbあるいはⅡbに近い症例を報告してきた．

　われわれが摘出標本レントゲノグラムから得たⅡbのX線像は，周辺非癌粘膜と比べて①areaが明るい，②area間溝がはっきりしていて幅が広い，③辺縁に接するとわずかな伸展不良がみられる，④バリウム付着異常，などであった．

　今回われわれは辺縁のわずかな伸展不良を手掛かりとして発見された純粋nbの症例を供覧する．

　We reported a case in which the findings of poorly stretched gastric wall and abnormal contour on roentgenograms led us to detect genuine Ⅱb type lesions.

　In general, frontal view of the cancer lesion is very difficult to take. The digree of the difficulty, however, depends on the specific type of the cancer.
