  • Look Inside

Adenomas of the stomach are histologically classified with their cell natures into two types, gastric proper epithelial type and intestinal metaplastic one.

Clinicopathological difference between adenomas of the intestinal metaplastic type of the stomach and large intestine has been reviewed.

Macroscopic appearances of the adenomas may be affected with growing mode of the adenomas and histological structures and peristalsis of the stomach and large intestine. The adenoma of the large intestine has a tendency to easily develop into pedunculated polyp.

It may be interpreted as one of histologically malignant findings that the horizontal extent to the mucosal surface is more predominant than the vertical extent to the one. Therefore, the pedunculated adenoma with moderate or severe atypia, whose tubuli replace the normal mucosa of the stalk, may be malignant.

The adenoma-carcinoma sequence of the large intestine has been compared with that of the intestinal metaplastic type of the stomach. Those two sequences may be thought to be not so much different.
