前庭部大彎側のⅡc型早期胃癌の1例―3年3ヵ月前のX線像，内視鏡像と対比して

A Case of Ⅱc Subtype Early Cancer of the Stomach Located on the Greater Curvature Side of the Antrum: Correlation of the endoscopic and x-ray findings with those taken 3 years and 3 months before

　近年胃疾患診断学の進歩は目ざましく，大彎側の病変も比較的容易に発見されるようになり，その部位の早期癌数も増加している．

　筆者らは，胃X線検査で発見した前庭部大彎側の比較的小さなⅡcの1例を経験したので，3年3カ月前のX線像，特に二重造影像，内視鏡像と対比して，またこの部位の病変のX線正面描写法について若干の考察を加えて報告する．

　A study is made of a case of small Ⅱc on the greater curvature of the antrum that was detected by x-ray with compression applied. Our study is centered on the best way of portraying its en face view together with the correlation of x-ray and endoscopic pictures taken 3 years and 3 months before with those obtained in recent examination.

　Besides a Ⅱc in the antrum, through x-ray checkup revealed an ulcer on the posterior wall of the upper body. However, as the ulcer was confirmed to have cicatrized, and was negative for cancer as confirmed by biopsy, gastrectomy was done leaving the ulcer scar untouched. It would have been better to excise the stomach including the ulcer scar, because it is now thought that cancer had already existed there 3 years and 3 months before.

　As it was, it is now necessary to follow up the patient strictly. This case has given us a lesson and a matter for reflection.
