胃の原発性形質細胞腫の1例

Primary Plasmacytoma of the Stomach, a Case Report

　胃の原発性形質細胞腫はきわめて稀なもので，Vasiliu（1939）が第1例目を報告して以来，34例が報告されているにすぎない．本邦では伝田ら¹⁾および吉井ら¹²⁾の2例のみである．著者らは本邦第3例目の，胃の原発性髄外性形質細胞腫を経験したが，これは，早期胃癌類似の肉眼型を呈しており，しかも悪性腫瘍性増殖と考えられた．このような肉眼的形態を示し，かっ悪性と考えられる例は，現在まで報告がないと思われるので報告しあわせて多少の文献的考察を加える．

　A report has been made of a case of primary plasmacytoma of the stomach encountered in a 28-year-old man. It is the third report of this kind in our country. Not only in findings of x-ray and endoscopy but also in those of gross observation it closely simulated Ⅱc+Ⅲ type early gastric cancer. In addition, the tumor cells showed pyroninophilia, atypicality and infiltration, so that we arrived at a diagnosis of malignant plasmacytoma. Such an early case has probably never been reported before both at home and abroad. Whether it be or not be an early pathological picture of primary gastric plasmacytoma is of interest to us. If it be so, preoperative diagnosis of this variety of malignant neoplasm may be possible on the basis of biopsied materials.
