直腸平滑筋腫の1例

Leiomyoma of the Rectum

　直腸に発生する平滑筋腫は比較的，稀なものである．最近，われわれは，下血と頑固な便秘を主訴として来院した71歳男の直腸平滑筋腫の1例を経験したので，若干の文献的考察を加えて報告する．

症例

　患　者：朝○清　71歳　男

　主　訴：下血，便秘

　既往歴：17歳の時痔瘻手術，55歳より現在まで高血圧治療中

　家族歴：特記することなし

　現病歴：1978年1月頃より頑固な便秘を訴え，売薬を服用していた．便通は数日ないし1週間に1度位の割合であった．4月20日，排便時，大量の下血（鮮血）があり，直腸癌を疑って4月24日本院を受診した．

　Myogenic tumor of the rectum is rare. A 71 year-old male was admitted to the Saiseikai Hospital in Nagasaki in April, 1978, complaining of constipation and blood in the stools. On rectal examination, a protruding tumor was noticed on the posterior wall of the rectum, 5cm above the anal sphincter. Biopsy revealed no malignancy of rectal mucosa. A submucosal tumor was suspected and amputation of the rectum was performed. The gross specimen showed a hemispherical tumor measuring 7.5×7.2×6.3 cm in size. The surface was round and smooth and covered by the rectal mucosa which showed erosion. The cut surface was of a dark-yellow color and elastic hard.

　Histologically, the tumor was diagnosed as leiomyoma and there was no evidence of malignancy.
