残胃に原発した細網肉腫の1例

Primary Reticulum Cell Sarcoma of the Remnant Stomach: a Case Report

　残胃には種々の病変が発生し，近年注目されるようになった．われわれは，胃潰瘍の手術後13年目に，その残胃に発生した細網肉腫を経験し，初回手術材料の検索が可能であったので若干の考察を加えてこれを報告する．

症例

　患者：71歳男性

　主訴：心窩部痛

　既往歴：58歳の時，某病院で胃潰瘍の診断の下に胃切除術を施行している．

　現病歴：1977年7月末より心窩部痛出現し，近医にて投薬治療を受けていたが改善しないため，精査，治療の目的で同年9月，当院へ入院．

　A case with reticulum cell sarcoma of the remnant stomach is reported.

　The patient was a 71-year-old man whose chief complaint was epigastric pain. Thirteen years ago, he had a gastrectomy under the diagnosis of gastric ulcer.

　By X-ray examination of the stomach, a Borrmann type 2 gastric cancer was initially suspected. However, endoscopy was performed and a diagnosis of malignant lymphoma was made in the biopsy specimens.

　Total gastorectomy and splenectomy were performed.

　On the resected specimen, there was a Borrmann type 2 like lesion in the anastomotic segment of the stomach. Histologically, the lesion was comfirmed as reticulum cell sarcoma of the stomach.
